Create and manage Lockouts on Assets and Work Orders

Introduction

Lockouts are records created to ensure a safe work environment. A Lockout contains a list of Lockout Points, which is the safety equipment required in a work area before it is safe to start work on a work order.

Maintenance Managers create and manage Lockout Points and Lockouts. Technicians, assigned to a Lockout, lock and unlock the Lockout Points on the Lockout.

Prerequisites

Setup for Lockout is completed.

Create a Lockout Point

A Lockout Point is a safety device you mount in a work area to ensure that maintenance work can be carried out in a safe way. Examples of Lockout Points are:

  • Lockout hasp
  • Valve lockout
  • Circuit breaker lockout

Safe Work Managers can work with Lockouts from the Safe Work Role Center.

When you create a Lockout, you add the required Lockout Points to the Lockout. On a Lockout Point card, the Number of Lockouts field and fields in the Dates section are automatically updated when the Lockout Point is used on a Lockout. The user can add Assets and Asset Locations on a Lockout Point card.

You can add a picture and documents to a Lockout Point. Pictures and documents are shown in FactBoxes to the right on a Lockout Point card. Select the "i" button to the right on the ribbon to see FactBoxes if they are not already visible. The handling of pictures and documents is very similar. You can take or import a picture. You can upload a document.

Refer to the test cases below for step-by-step procedures on how to create a Lockout Point and import/change/take pictures on a Lockout Point card.

Lockout Point from the Asset

You can create the Lockout Point for the Asset directly from its card, or from the Asset Tree.

The Create Lockout Point action creates a lockout point from the asset and opens the card of the created record:

  • Description of the created lockout point is taken from the Asset Description.
  • Connected to Type on the lockout point is set up as Asset.
  • Connected to No. on the lockout point is set up as Asset No. from which the point is created.
  • Asset Location Code is taken from the Asset.
  • On the Lines - the line with the type Asset and the No. taken from the Asset is created.

Location of the Lockout Point

On the Lockout Point card, you can set up information about the Location of the Lockout Point.

In the Asset Location Code field, you can specify the code of the location. This is the same set of locations that you can use on the Asset. GPS Coordinates are taken from the Location Code every time you're changing the value in the Asset Location Code field (if coordinates are added to the location). GPS Coordinates can always be changed manually. You can also add some additional information about the location in the Location Description field.

Creating a Lockout Point

Data Requirements

  1. Number sequence for Lockout Points exists.
  2. The current user is set up as a Safe Work User.

Steps 5

  1. On your role center, search for "point".

  2. Select Lockout Points in the list.

  3. In the Lockout Points list, select New.

  4. Enter a name for the Lockout Point in the Description field.

  5. Optional: In the State field, select a state if states are available.

The user can create a Lockout Point.

  1. At least one Lockout Point exists (with specified description).
  2. An existing jpg/png file.
  3. A Lockout Point card is open.

Steps 5

  1. Find the Picture FactBox on the right side of the screen.

  2. Select the arrow next to Picture.

  3. Select Import.

  4. Select Choose.

  5. Select the file.

Expected

The user can add a picture to a Lockout Point.

Changing Lockout Point Picture

Data Requirements

  1. At least one Lockout Point (with picture) exists.
  2. An existing jpg/png file.
  3. A Lockout Point card is open.

Steps 6

  1. Find Picture in FactBoxes.

  2. Select the arrow next to Picture.

  3. Select Import.

  4. Select Yes.

  5. Select Choose...

  6. Select the file.

Expected

The user can change a picture on a Lockout Point.

Taking Lockout Point Picture

Data Requirements

  1. The user is using a mobile device with a camera.
  2. At least one Lockout Point exists in the database.
  3. A Lockout Point card is open.

Steps 5

  1. Find Picture in FactBoxes.

  2. Select the arrow next to Picture.

  3. Select Take.

  4. Take a picture.

  5. Select Use.

Expected

The user can take a picture with a mobile device and add it to a Lockout Point.

Print Lockout Point Label

On the Lockout Point Card, you can print a label with a barcode or QR code.

On the Lockout Point Card, look for Print Label. Select it to print the label for the current lockout point or multiple others. If you clear the No. filter and leave it blank, labels for all lockout points will be printed. By default, the printout template has a layout of 4 cm x 1.5 cm (1.57" x 0.59") and the code is Code39.

There are four options of coding: Code39, Code128, QR Code, and Aztec Code. Each of these codes requires specific font to be used:

Lockouts List

The Lockouts list shows an overview of all Lockouts and displays some details for each record.

  • To open the Lockouts list, search for "lockout" on your role center, and select Lockouts.

Use the filter in the Lockouts list to show Lockouts with at specific status or Lockouts assigned to you. Refer to the test case below for a step-by-step procedure on how to select a filter in the list.

Selecting a Filter in the Lockouts List

Data Requirements

  1. At least one Lockout (with any status) exists.

Steps 3

  1. On your role center, search for "Lockouts".

  2. Select Lockouts in the list.

  3. In the Lockouts list, the filter is located in the upper-left corner, next to Lockouts: in the ribbon. The default selection is "All".

    Open the drop-down list and select a filter.

Expected

The user can filter the view in the Lockouts list.

Create a Lockout

A Lockout is a record that describes the safety measures required to ensure that maintenance work can be carried out in a safe way on machinery and other types of equipment. Lockout examples are

  • Cut off power
  • Secure gas valve
  • Secure fluid valve
  • Lock down a work center

When you create a Lockout, you add the required Lockout Points to the Lockout. The Lockout status is automatically updated, depending on the actions taken on the Lockout. Here is a status overview:

Status Description / related action
Draft The lockout is created.
Pending Lock The lockout is approved.
Pending Unlock All Lockout Points are marked "Locked" meaning they are installed in the work area.
Finished All Lockout Points are marked "Unlocked" meaning maintenance work in the secured area is completed.

Refer to the test case below for a step-by-step procedure on how to create a Lockout.

It is possible to copy a Lockout record if it is approved. This is useful if you want to use the same Lockout for another maintenance job.

  1. On the Lockouts list, open the Lockout card you want to copy.
  2. Select Process > Copy Lockout.
  3. Confirm copy. The following source information is copied: Description, Assigned To, and Lockout Points.

Creating a Lockout

Data Requirements

The user can create a Lockout and add Lockout Points to the Lockout.

Approve a Lockout

When a Lockout is created and the required data and Lockout Points have been added, you can approve it.

  1. Open a Lockout from the Lockouts list.
  2. On the Lockout card, select Process > Approve.
  3. Confirm Approval. The Status field is updated to "Pending Lock".

If you want to revert the Lockout to status "Draft", that can only be done as long as no Lockout Points have been locked. To change the Lockout status back to "Draft", select Process > Change To Draft, and confirm the change. This procedure may be relevant if, for example, you want to change the planned start/end dates.

When you have approved a Lockout, next step is to mount the Lockout Point(s) in the work area and update the status.

Mount/Lock a Lockout Point

When a Lockout is approved, and the planned start date approaches, next step is to physically mount Lockout Points in the work area. When the Lockout Points are mounted, you mark them on the Lockout as "Locked" to indicate that maintenance work can start in the work area.

The user selected in the Assigned To field can lock the Lockout Points.

  1. Open the Lockout from the Lockouts list.
  2. On the Lockout card > Lockout Points section, select the Locked check box for the Lockout Points you have mounted in the work area.
  3. Confirm that Lockout Points have been locked.
  4. When all Lockout Points are locked, the Status field on the Lockout is updated to "Pending Unlock", and the Actual Start Date field is filled out.

When maintenance work is completed in the secured work area, next step is to unlock the Lockout Points.

Remove/Unlock a Lockout Point

When maintenance work is completed in the work area that you created a Lockout for, you remove the Lockout Points from the work area and unlock them on the Lockout.

The user selected in the Assigned To field can unlock Lockout Points.

  1. Open the Lockout from the Lockouts list.
  2. On the Lockout card > Lockout Points section, select the Unlocked check box for the Lockout Points you have removed from the work area.
  3. Confirm that Lockout Points have been unlocked.
  4. When all Lockout Points are unlocked, the Status field on the Lockout is updated to "Finished", and the Actual End Date field is filled out.

The Lockout is completed.

Create Lockout from Lockout Point

You can easily create the lockout with an already locked Lockout Point directly from the Lockout Points list (or lockout point card) by using the Lock action.

  1. Select Lock.
  2. Specify what the Planned End Time is of the Lockout that you are about to create. You can also specify the Work Order No. and add some Remarks.
  3. After confirming the process you will be notified about the created Lockout.
  4. The created document has:
    • Description: taken from the Lockout Point.
    • Assigned To: set up as the user who created the lockout.
    • Status: Pending Lockout Removal (approval is skipped).
    • Planned Start Date/Actual Start Date: set up as the date time of the moment when you confirmed the creation.
    • Planned End Date: set up created by you in the previous step.
    • Remarks: added in the previous step.
    • Lockout Points line with the Lockout Point from which you created a lockout and with activated lock.
  5. Now you can process this lockout directly from its card, but you can also open the Lockout Points list again and unlock the lockout point directly there (or its card).
  6. When you use the Unlock action, you'll be notified about the removal of the lock from the specific lockout. This finishes the lockout.

Note:

  • Only the user, who created a lockout directly, can unlock specific lockout points with these actions. If for some reason someone else must post the unlocking, change the Assigned To person on the lockout and process the document manually.
  • The specific user can use actions that are available to that user:
    • the Lock action is enabled if this lockout point is not currently locked by this user.
    • the Unlock action is enabled if the lockout point was locked by the current user and still waiting to be unlocked.

Print Lockout Tag

On the Lockout card, you can print the tag for the lockout. This allows you to print the information about the lockout in a form of a tag, that should be placed alongside the lock. The report automatically generates a tag for each lockout point in the lockout.

  1. On the Lockout card, use Tell Me to search for the Print Tag or go to the Print group on the ribbon and select Print Tag.
  2. A modal window with printing options is shown where you can choose for which lockout the tag will be printed, and for which lockout points from the lockout.

The tag from the default Word layout looks like the one in the example below, but you can customize it to suit your needs.

Event Log

The Event Log gives you an overview all the registrations made on a Lockout.

  1. Open a Lockout from the Lockouts list.
  2. On the Lockout card, select History > Show Log.
  3. The Event Log contains all registrations and updates made on the Lockout.

Note: On your role center, you can search for "safe work" and select Safe Work Event Log. That log contains all registrations created on all modules that are included in the Dynaway Safe Work app.

Locking History of the Lockout Point

You can inspect the locking history of any lockout point. To do this, select the Locking History action on the Lockout Point card or on the Lockout Points list.

Locking History opens a list of all lines (from the lockouts) that were locked in the past. Here you can:

  • Check the number of the original document.
  • Who locked them and when.
  • who unlocked them and when.
  • See if the line was created manually, directly from the Lockout Point, automaticallt from a Work Order, or API.

Important: list represents all existing lines on the Lockout documents. If you delete for some reason lockouts - lines will be also deleted.

Lockout Automation

A Lockout Automation Setup page created.

Based on the setup on this page - any new Work Order created from Work Order Plan (so One-offs are not supporting this solution) will trigger lockout with proper lockout points based on the setup.

From given picture.2

  • any Work Order will trigger Lockout with D/00005 lockout point
  • any Work Order and with Asset Location (H1.MH) will trigger Lockout with D/00005 and D/00004 lockout point
  • any Work Order and with Asset Category (UV LAMPS) will trigger Lockout with D/00005 and D/00003 lockout point
  • etc.

When Lockout is triggered by Work Order - Work Order No. is visible on the Lockout card.
